Scheduling Policies
Pre-appointment details
Once you initiate your booking, you will be directed to our scheduling and intake portal. There, you will be able to schedule your exam, provide your health history, insurance, and contact information. Please fill out all information categories as completely as possible so that you are ready for your exam. Lastly, please arrive 15 minutes prior to your exam time.
Cancellation policies
We do realize, from time to time, some appointments do need to be rescheduled or cancelled. If you are unable to make your appointment, please let us know 24 hours in advance of your appointment. With more than three consecutive no-shows or cancellations, you will not be offered any appointment times thereafter. We will see you on a walk-in basis only, and be mindful that scheduled appointments will have priority.
Payment policies
All fees and copays will be collected on the day of service. For insurances, we will do our best to identify your insurance benefits prior to your exam with the information you provide, however you are ultimately responsible for your own benefits.
